By using the Medical Mileage tool ("the Service"), you agree to these Terms. If you do not agree, do not use the Service.
What the Service is — and is not
The Service is a free self-help tool that helps California injured workers track medical-travel mileage and fill out their own state reimbursement forms. It is not legal advice. Using it does not create an attorney–client relationship, and it does not make The Clayton Perry Law Office for Injured Workers, APC your attorneys. The mileage and amounts it produces are estimates based on the information you enter. You are responsible for reviewing every form and confirming it is accurate before you sign it.
Your responsibilities
You agree to provide accurate information and to use the Service only for your own lawful workers' compensation claim. When you sign a form, you are attesting that the information on it is true. Submitting a false or fraudulent claim is a crime under California law.

The Service sends login codes by SMS. Message and data rates may apply; see the Privacy Policy for details and opt-out.
No warranty; limitation of liability
The Service is provided "as is," without warranties of any kind.
Changes
We may update these Terms; continued use after a change means you accept the updated Terms.
